All,
In all my years of shooting I have never seen what my new toy did today!
I was using Rem 223 ammo in my CMMG 5.56 and it did this twice.

I have used a safe round to show you what I had to deal with.
Can someone explain why this happened, I dont like spending 10 min trying to get my Round out.

I hope this is a easy fix!

Did it happen with the same magazine? If so check the feed lips on the mag, they might be bent outwards a little bit allowing it to try and feed two rounds at the same time forcing one of them above the bolt.
 
I agree with what has been posted so far - try a different magazine and see if that fixes the problem. A buddy of mine had the same issue with his brand new M&P, and it turned out it was the 45-round ProMag he was using. Ran 20 rounds through his using my PMAG with zero malfunctions.
 
I agree about it being the followers but Wow I have never seen that ever happen even with shitty GI followers. I live in NY and have purchased a number of old 30 round GI magazines and found the followers to cause a bunch of problems. If you have GI magazines just upgrade the follower with Magpuls anti-tilt. It's like $5 for a pack of 3 from midwayusa. :)
 
I used to be an armorer when I was in the Army so I have seen that a few times out at the ranges, it can be a real pain in the ass to get out. If it happens again and the gun is hot, set it down and let it cool off for 10 minutes before you try and pry it out. You dont want a round cooking off while you're screwing with it!
