shifting issues (clutch?)

So my clutch has slipped some in 5th gear around 4500 rpm for a while now. Yesterday I was riding and my shift attempts would not take the first attempt. It took two times to get it to shift gears. I rode it home. When turning on to my street it became even more difficult. I can now only shift from 1st to 2nd. Can get in to neutral without issue. I pulled the primary and clutch assembly. Everything looks normal way first glance, but I'm not sure exactly what to look for. Any suggestions on where to go from here? Am I looking in the wrong place? Could it all just have been an adjustment issue? I was pretty sure I had it adjusted correctly.
 
Are you adjusting the clutch per the service manual? At the case first then at the lever/cable?

I'd also recommend checking the lips on the primary cover where the ramp sits. If they were starting to break, that may have given you those results due to improper clutch disengagement.

Depending on the year of your bike, you may want to upgrade to the newer Teflon clutch cable and replace all seals and o-rings associated with the primary cover.

Decided lunaticfringe is right. Ordered plates today. Will be putting in new parts next week and let you guys know the outcome
 
I bought one with the clutch. Got it all together yesterday and everything seems to be good to go. Went with an energy one clutch kit. So far I'm happy. Price was right and seems to be good quality product. Good customer care too. Couldn't quite figure out the tool at first. Callers them up and the walked me through getting it attached and the project started
