Shift pedal fell off! Where to find part?

Good evening Gentlemen,

I was riding to work the other day and as I was exiting the freeway I started to down****- or was about to. I tried to shift and noticed that there was nothing there!! I looked down and sure enough the shift pedal was dangling in the wind. Needless to say it was a fun experience keeping the bike in top gear while starting from a dead stop at several red lights.

I was able to find a replacement bolt from the hardware store, but I'm still missing the metal sleeve or spacer that the bolt slides through. I've attached a picture of what I'm looking for. Does anyone know the exact dimensions of this part or where I can buy it? I'd prefer to not purchase and entire shifter kit since I have everything else.

item you have circled in red is called "pivot bolt sleeve". part # N0599.02A8A available thru any dealer.
NOTE: if your pair of item #22 is missing you'll also need them. bolt/sleeve will NOT function without them. see schematic.

@Paladinew: My shifter pivot bolt unraveled on me twice, red Loctite is needed. Before you reinstall the bolt, take some red Loctite, dab it on a fine pointed brush, and coat the inside threads where the pivot bolt resides.

I forgot to mention its a standard length and diameter to be able to replace the stock bolt and sleeve assembly with just a shouldered bolt. I found the right bolt at a True Value hardware store.
